Job DescriptionJob DescriptionThe Licensed Practical Nurse (LPN) will provide skilled nursing care primarily in home health care settings. Working under the direct supervision of a registered nurse (RN) or physician, the LPN will manage a low patient caseload of 1 to 5 individuals, ensuring comprehensive and compassionate care. This role offers clear pathways for professional development and advancement within the healthcare team.
Responsibilities
Deliver direct patient care in home health environments
Administer medications accurately and safely
Monitor vital signs and report any changes
Perform wound care and dressing changes
Provide patient education to support health management
Assist RNs with clinical duties and care coordination
Implement infection control protocols
Contribute to individualized care planning
Maintain clear and effective communication with patients and healthcare providers
Preferred Qualifications
Minimum of 1 year nursing experience
