About This Role
The Nurse Manager I, Continuum of Care is responsible for overseeing patient care delivery systems and practice standards, collaborating with healthcare teams, and ensuring optimal patient care across various locations. This role is ideal for individuals who excel in leadership and operational management within a healthcare setting.

What You'll Do
Implement best practice models and patient care delivery systems.
Investigate and resolve patient/family/member concerns regarding care and services.
Collaborate with physicians and healthcare providers to achieve optimal patient care.
Monitor resource needs in assigned clinical areas for appropriate utilization.
Ensure the safe environment for patients and employees.
Provide clinical supervision and maintain clinical expertise.
Delegate tasks aligned to scope of practice.
Manage designated work unit by translating business plans into tactical actions.
Drive implementation of action items for continuum of care operations.
Develop strategic partnerships with physicians and manage complex work streams.
