Job description
St. Joseph’s Health seeks a Nurse Navigator to serve as the clinical anchor within Central Referral Management.
You will triage referrals by acuity, resolve clinical questions, and elevate urgent cases to ensure timely patient access while maintaining safety and quality. The role partners with referral representatives, specialty practices, and patients to remove barriers to care and shorten the time from referral to encounter across service lines.
