Job description
Beth Israel Lahey Health in Burlington, MA is seeking a Registered Nurse to identify patients eligible for the Hospital at Home program and coordinate safe transitions from the Emergency Department to HaH, community placements, or other care.
You will collaborate with ED clinicians, Hospital Medicine, and outpatient providers, evaluate eligibility, and ensure proper documentation and patient education throughout the disposition process.
