Job description
Courtyard Rehab is seeking a Nurse Navigator to provide timely clinical assessment, intervention, and care coordination for nursing-home residents with changes in condition. The role aims to identify and manage potentially avoidable changes while ensuring timely transfer for those needing higher levels of care.
You will collaborate with nursing leadership, physicians, EMS, and hospitals to improve outcomes and reduce avoidable transfers and readmissions.
