Job description

Job Description

The Nurse Navigator is the clinical anchor embedded within the Central Referral Management team, working side-by-side with referral representatives to convert incoming referrals into scheduled, completed patient encounters across St. Joseph's Health service lines. While referral representatives manage the operational workflow of intake, data entry, and scheduling logistics, the Nurse Navigator applies clinical judgment to triage referrals by acuity and appropriateness, resolve clinical questions that stall conversion, and elevate urgent or complex cases for timely intervention. The role bridges referring providers, receiving specialty practices, and patients to remove clinical barriers to care, shortening the time between referral submission and patient access while safeguarding quality and safety.
