Job description
The Nurse Navigator at Wellstar Health System in Augusta, GA is a licensed RN who provides individualized support to patients, families and caregivers to help overcome barriers to care across the cancer continuum. The role interfaces with patients in person, by phone, and electronically to promote timely access to care and adherence to the care plan.
