Job description
Duke Health is seeking a Nurse Navigator (RN) to coordinate care for high-risk patients across the care continuum at Duke Regional Hospital in Durham, NC. The NN will triage referrals, advocate for patients, and provide education while collaborating with a multidisciplinary team.
Requires BSN with 3 years of relevant nursing experience; MSN or navigator training preferred. May include relocation assistance up to $8,000 and on-site work arrangements.
