Nurse Practitioner / Physician Assistant – Primary Care / Endocrinology
Location: Concord, CA 94520
Job Type: Full-time | Pay: $65.00 – $70.00 per hour
Expected Hours: 40 per week | Schedule: Monday – Friday (No nights or weekends)

As we continue to grow, we are seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our outpatient team with a focus on Primary Care and Endocrinology .
Position Summary
The ideal candidate will provide full-spectrum primary and preventative care with a focus on chronic disease management , particularly for patients with diabetes and endocrine disorders. There is also an opening for Allergy & Sleep; these both include medication management, ongoing monitoring, and coordination of care with our internal care team and external specialists.
You will build expertise in managing complex endocrine patients while delivering general primary care in a supportive, team-based environment.
Key Responsibilities
Perform comprehensive physical examinations and diagnostic evaluations
Prescribe medications and implement care plans
Monitor and manage chronic conditions, including diabetes, hypertension, and hyperlipidemia
Collaborate with physicians and specialists on care coordination
Educate patients on preventative care and wellness
Accurately document care using our EMR system (Epic)
Qualifications
Education: Master of Science Degree from an accredited Nurse Practitioner or Physician Assistant program
Current California licensure (NP or PA-C)
Preferred Experience: Experience in outpatient primary care
Background working with adult and geriatric patients
Familiarity with Epic EMR system
Skills: Excellent communication and interpersonal abilities
Strong organizational and clinical decision-making skillsAbility to work independently and collaboratively within a team
