Job description
Mid-Level Practitioner (PA or NP) – Wound and Primary Care Provider
We are seeking a skilled and compassionate Provider to join our full‑time Wound and Primary Care team. As a Provider, you will be responsible for providing specialized care to patients with primary care needs as well as wounds, ensuring their comfort and promoting healing.
Responsibilities
Perform comprehensive assessments of patients with or without wounds, including diagnostic evaluations.
Develop and implement individualized wound and primary care plans based on assessment findings.
Provide wound care treatments at the patient's bedside, such as dressing changes, debridement, and other procedures.
Provide primary care services to patients in an assisted‑living setting.
Monitor and evaluate the progress of wound healing.
Collaborate with interdisciplinary healthcare team members to coordinate patient care.
Educate patients and their families on wound‑care techniques and prevention strategies.
Maintain accurate and up‑to‑date documentation of patient assessments, interventions, and outcomes.
Perform rounds in long‑term care facilities, assisted‑living communities, office, and occasionally home visits.
Qualifications
Must hold an active Physician Assistant or Nurse Practitioner license in Idaho.
Experience of 1–3 years as a provider is preferred, but we will train the right new graduate.
Strong knowledge and experience in wound‑care management preferred but not required.
Ability to triage patients effectively in emergency situations.
Strong communication skills.
Experience in post‑acute care settings (Assisted Living or Skilled Nursing) recommended but not required.
Driving is required; dependable vehicle and a valid driver’s license are musts.
Mileage is reimbursed.
Salary is dependent on experience.
