nPosition Summary
nThe Nurse Practitioner will provide comprehensive primary care services to adult patients, including preventive screenings, health assessments, diagnosis, treatment, and ongoing management of acute and chronic medical conditions. As a key member of our clinical team, you will work collaboratively with physicians, nurses, and other healthcare professionals to ensure exceptional patient care.
nKey Responsibilities
nnnConduct comprehensive health assessments, screenings, and physical examinations for adult patients.
n
nnDiagnose and treat acute, chronic, and preventive healthcare needs.
n
nnDevelop and implement individualized treatment plans based on clinical findings.
n
nnProvide patient education and counseling to promote health and wellness.
n
nnCoordinate care with specialists and other healthcare providers as needed.
n
nnParticipate in regular chart reviews and quality improvement activities.
n
nnMaintain an active clinical practice consistent with professional standards and BMS policies.
n
nnCollaborate with the medical team to ensure continuity and quality of patient care.
n
nnSupport compliance with Managed Care/HMO requirements, SBHC contracts, and Federal and New York State healthcare regulations.
n
nnContribute to the development and implementation of clinical policies and procedures.
n
nnRegularly consult with clinical leadership, including the Assistant Medical Director and Nurse Manager, regarding patient care and clinical operations.

n nRequirements
nQualifications
nRequired:
nnnCurrent New York State license as aNurse Practitioner.
n
nnMinimum oftwo (2) years of clinical experienceas a Family Nurse Practitioner or Adult Medicine Nurse Practitioner.
n
nnStrong clinical assessment, diagnostic, and patient management skills.
n
nnExcellent communication and interpersonal abilities.
n
