Job description
Role:
The Nurse Practitioner provides effective, equitable, value-based primary care to adult Medicare patients, focusing on keeping them healthy and out of the hospital. They work independently to conduct office visits, wellness exams, and care coordination, building trust and meaningful relationships with patients. The role involves collaborating with a multidisciplinary team, participating in population health initiatives, educating patients, and conducting home visits as needed.
Responsibilities:
Deliver exceptional primary care
manage routine and acute visits
perform wellness assessments
coordinate care with other providers
lead population health efforts
assist with triage and outreach
and support promotional activities
Qualifications:
Master's or DNP degree
national certification in primary care specialty
active NP license
DEA license
US work authorization
Preferred experience in primary care
understanding of HCC and ICD-10 coding
teamwork
population health focus
bilingual skills in Spanish
Additional Details:
Full-time position, approximately 40 hours/week, with a competitive salary range.
