nRequired Qualifications
nYour Role & Responsibilities
nnnYou will provide comprehensive care to seniors following clinical standards.
n
nnYou will collaborate daily with your care team to ensure coordinated, high-quality care.
n
nnYou will anage referrals, hospital/SNF coordination, DME, and home health services.
n
nnYou wil participate in daily huddles.
n
nnDocument care accurately and efficiently with support from quality-based coders.
n
nnEnsure compliance with all licensing and accreditation requirements.

n nWhat We're Looking For
nRequired:
nnnMaster's Degree in nursing or completion of a physician assistant program
n
nnBoard certification
n
nnActive, unrestricted NP or PA license in the state of practice.
n
nnOne year of experience in primary care
n
nnExperience with electronic medical records
n
n nIf selected for this role, you will be required to be screened for TB.
nPreferred Qualifications
nnnActive DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
n
nnFamiliarity with HEDIS quality indicators and Medicare guidelines.

nThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
n$123,800 - $170,300 per year
nThis job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
