Your Role Will Include:
Comprehensive Patient Care:
Conduct physical exams, assess symptoms, and diagnose medical conditions
Order and interpret diagnostic tests (blood work, imaging, etc.)
Develop and implement personalized treatment plans tailored to each patient
Prescribe medications, therapies, and interventions to manage both acute and chronic health issues
Provide disease management and follow-up care to ensure optimal patient health
Offer preventive care such as immunizations, screenings, and wellness check-ups
Patient Education & Support:
Educate patients and their families about health conditions, treatment options, and lifestyle changes
Provide guidance on nutrition, self-care, and disease prevention for improved well-being
Empower patients to take control of their health with ongoing education and support
Collaboration with Healthcare Teams:
Work closely with physicians, specialists, and other healthcare providers to coordinate comprehensive care
Participate in case discussions and contribute insights to patient management plans
Provide guidance and support to other clinical staff members as needed
Ongoing Patient Monitoring & Adjustments:
Regularly assess and monitor patient progress, adjusting care plans as necessary
Ensure all documentation is accurate and up-to-date in electronic health records (EHR)
Advocate for patients by reviewing feedback and ensuring continuous improvement in care
Qualifications:
Master's degree in nursing from an accredited Nurse Practitioner program
Active Michigan Nurse Practitioner license
Board Certification as a Nurse Practitioner (e.g., Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, etc.)
Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications preferred
Strong clinical skills with the ability to assess, diagnose, and treat a variety of conditions
Excellent communication and interpersonal skills for patient engagement and teamwork
Ability to work independently while also collaborating with a healthcare team
Empathetic, patient-centered approach to care
