A mission-driven community health organization providing high-quality, patient-centered care to a diverse patient population in the Bronx is looking for a Nurse Practitioner to join their team.
Full-Time | Flexible Schedule

About the Opportunity
The ideal NP will have 2–3+ years of primary care experience and be comfortable providing comprehensive care in a community-based setting.
You’ll work as part of a collaborative, multidisciplinary team providing accessible, respectful, and evidence-based primary care across a diverse patient population.
Schedule:
3 daytime shifts: 8:30 AM–4:30 PM
2 evening shifts: 12:00 PM–8:00 PM
Optional Saturday: 9:00 AM–3:00 PM
Some flexibility in scheduling, including the ability to have input into evening shifts
What You’ll Do
Provide comprehensive primary care services across the lifespan
Conduct evaluations, examinations, diagnoses, and treatment
Manage follow-up care, test results, referrals, and ongoing patient needs
Provide patient-centered care in a respectful and inclusive environment
Document clinical care in EPIC
Collaborate with physicians, nurses, behavioral health professionals, and other members of the care team
Participate in quality improvement, case conferences, and professional development xxzpjkt
Qualifications
Current New York State NP license/registration
Current DEA registration
Current CPR certification
2–3+ years of primary care experience strongly preferred
Community health experience a plus
HIV experience strongly preferred
Spanish/English bilingual skills are a plus
