Job description
Nurse Practitioner
Carease Health is seeking a dedicated and experienced Nurse Practitioner (NP) to join our team and deliver high-quality preventative care to residents in skilled nursing facilities. As a key member of our hybrid team, you will combine on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.
Responsibilities:
Provide comprehensive preventative care to residents in skilled nursing facilities, utilizing Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) strategies
Build strong relationships with facility staff and patients, maintaining a patient-centered approach and collaborative teamwork
Document patient care efficiently, utilizing a dedicated scribe team for support
Participate in ongoing education and training to stay current with industry developments and best practices
Requirements:
Active NP or PA license in the state of practice
1+ year of experience in Geriatric care and documentation review
Excellent communication, collaboration, and teamwork skills
Strong patient-centered approach and ability to work effectively in a fast-paced environment
