nWe are seeking a compassionate Nurse Practitioner to join our growing team of dedicated professionals at the Albany Med Center of Excellence for Alzheimer’s Disease! This is a great opportunity to join our multidisciplinary team to make a profound impact on advancing care for dementia patients in the region.
nThe CEAD Nurse Practitioner will work in close collaboration with the MDs, nurses, social workers, and other team members to deliver compassionate and top quality patient centered care for our dementia patients. The Nurse Practitioner will work with physicians to evaluate and manage all clinical patient needs. The Nurse Practitioner will contribute to programmatic development, outreach, patient navigation, and quality initiatives of the department. The Albany Med Center of Excellence for Alzheimer’s Disease provides advanced and subspecialized dementia care for patients throughout the region including the monoclonal antibody infusion treatments as well as comprehensive case management support.
nEssential Duties and Responsibilities include:
nnnAssesses, plans, implements and evaluates care for individual patients
n
nnAssesses patient/family needs and provides education
n
nnPromotes wellness and recommends community resources to meet patient and family needs
n
nnPromotes/provides staff education
n
nnIncorporates evidence-based research into clinical practice
n
nnKnowledgeable of reimbursement procedures and documentation requirements
n
nnWith the support of the Center social workers, provides case management for patients and their families
n
nnServes as a resource person for the nursing staff
n
nnMay mentor new APPs
n
nnWorks in collaboration with other services (radiology, apheresis, laboratory) to coordinate care for dementia patients undergoing monoclonal antibody infusion treatments
n
n nQualifications
nnnMaster's Degree of an accredited Nurse Practitioner program - required
n
nnAble to quickly master all aspects of a computerized medical recordkeeping and billing system.
n
nnMust project a professional image and earn the confidence of others.
n
nnExcellent interpersonal, verbal, and written skills.
n
nnMust be HIPAA compliant at all times.
n
nnNP - Nurse Practitioner – Licensed Upon Hire - required
n
nnATLS - Advanced Trauma Life Support Upon Hire - required
n
nnBCLS - Basic Life Support Upon Hire - required
n
n nMust be able to perform the essential functions of the position in a clinical environment, including, but not limited to, standing, walking, lifting, bending, kneeling, and reaching. Requires sufficient visual, auditory, and manual dexterity abilities to assess patients, provide direct patient care, operate medical equipment, and complete documentation.
