Job description
Ponos Care, a physician-driven care model, seeks a licensed Nurse Practitioner to oversee a chronic disease management population in Virginia. This hybrid role combines telehealth with in-home visits within a 30-mile radius of the Northwest/Southwest counties, collaborating with physicians, social workers and care coordinators.
