Job description
Ponos Management Inc. in Virginia seeks a Certified Nurse Practitioner to oversee a chronic disease management program for hematology, rheumatology and gastroenterology patients, in a hybrid model with telehealth and in-home visits within a 30-mile radius of targeted counties.
