We are seeking a compassionate and motivatedNursePractitioner to provide wound care services to patients in skillednursing facilities and nursing homes across the Rome, Utica, Syracuse, andWatertown areas. This is an excellent opportunity for a provider who enjoysworking independently, values flexibility, and wants to make a meaningfulimpact in long-term care settings.
We are open tofull-time or part-time candidates.Part-time providers may cover a portion of the facilities based on availabilityand preference.
Responsibilities
Providewound care evaluations and follow-up visits in nursing home settings
Assess,diagnose, and manage acute and chronic wounds
Developand implement individualized treatment plans
Collaboratewith nursing facility staff and interdisciplinary care teams
Documentpatient assessments, progress notes, and treatment recommendations
Educatestaff on wound prevention, healing, and best practices
Ensurecompliance with applicable standards of care and documentationrequirements
Qualifications
Strongclinical assessment and patient care skills
Woundcare experience preferred, but not required
Willingnessto train in wound care for candidates with the right background andinterest
Excellentcommunication and time management skills
Abilityto travel between assigned facilities
Annualsalary range: $140,000–$170,000 , based on experience and schedule
Full-timeand part-time opportunities available
Opportunityto work independently in a rewarding specialty
