Job description
As a Nurse Practitioner Trainer at HarmonyCares, you’ll educate and mentor Nurse Practitioners who provide home-based primary and palliative care to medically complex, home-limited patients. You will onboard new NPs, deliver hands-on clinical training in patient homes, and reinforce best practices in assessment, chronic disease management, telehealth, and care coordination. Partnering with an interdisciplinary team, you’ll refine training materials, support quality initiatives, and ensure NPs deliver compassionate, compliant, and outcome-driven care aligned with HarmonyCares’ mission.
