Hoag Health, the top-ranked health system in Orange County, CA is seeking a Nurse Practitioner to join our growing Memory and Cognitive Disorders (MCD) program in Newport Beach, CA. This role comes at an exciting moment of growth: the MCD program is moving into a newly inaugurated Center for Brain Health, launched with a transformative $50 million donation. The Center will be a destination for comprehensive dementia care, collaboration, and innovation.
Role Details
Schedule: Full-time, Monday-Friday
Setting: Outpatient-based position
Location: Newport Beach, CA and Irvine, CA
Conditions: Memory & Cognitive disorders (Dementia, Alzheimer's Lewy Body, Frontotemporal, Vascular)
Team: Cognitive & Memory Physician, Nurse Practitioner, Administrative support staff
EMR: Epic
Key Responsibilities Provide outpatient specialty care for patients with dementia and related neurodegenerative conditions, including Alzheimer's disease, frontotemporal dementia (FTD), Lewy body dementia, mild cognitive impairment, and other complex cognitive presentations
Collaborate closely with Cognitive & Memory Physician and other specialists to deliver individualized evaluation and treatment plans
Support patients and families with clear communication, counseling, and longitudinal care planning

Requirements: Nurse Practitioners or Physician Assistants considered
3-5 years of experience as a Nurse Practitioner or Physician Assistant managing Neurology or Geriatric patients
Demonstrated experience managing dementia and neurodegenerative disease
Graduate of an accredited Nurse Practitioner training program
California Nurse Practitioner License (active and in good standing)
Strong communication and interpersonal skills and a commitment to compassionate, family-centered care
Ability to thrive in a multidisciplinary, team-based environment
