The program brings together providers, nurses, behavioral health specialists, and other care team members to deliver coordinated, patient-centered care focused on improving health outcomes, enhancing access to services, and supporting long-term health management.We are seeking a full-time, 40-hour Nurse Practitioner to join the Compass Plus team, caring for a panel of approximately 120 patients throughout the Northern Massachusetts region, including but not limited to Lawrence, Lowell, Peabody, and Newburyport. The schedule for this role is Monday through Friday, 8:30 AM to 5:00 PM, with no nights, weekends, or on-call responsibilities.This is a primarily remote position that requires regular local travel to patients' homes, primary care offices, and other community-based settings. The selected candidate will receive field-based onboarding and training, as well as all necessary equipment, including a laptop, cell phone, GPS safety device, and patient care supplies. In addition, attendance at a monthly in-person team meeting, as well as other occasional trainings and meetings at our Assembly Row office in Somerville, is required.SummaryThe Nurse Practitioner (NP) is a licensed provider.
