n nYour Role & Responsibilities

nnnYou will provide comprehensive care to seniors following clinical standards.

n

nnYou will collaborate daily with your care team to ensure coordinated, high-quality care.

n

nnYou will manage referrals, hospital/SNF coordination, DME, and home health services.

n

nnYou will document care with support from quality-based coders.

n

nnYou will participate in on-call rotation and support growth plans at your center.

n

nnYou will have experience providing value-based care.

n nUse your skills to make an impact

nWhat We're Looking For

nRequired:

nnnMaster's Degree in Nursing or completion of a PA program with board certification.

n

nnActive, unrestricted NP or PA license in the state of practice.

n

nnExperience in value-based care, primary care or Medicare Advantage settings.

n

n nPreferred:

nnnBilingual (English/Spanish)

n

nnActive DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers

n

nnFamiliarity with HEDIS quality indicators and Medicare guidelines.

nPay Range

nThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

n$123,800 - $170,300 per year

nThis job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
