Job description

Connected Cardiovascular Care Associates (C3) is seeking an experienced Nurse Practitioner or Physician Assistant to join our growing cardiology practice serving the Heath and Dallas communities. This role offers a balanced mix of outpatient clinic care and light inpatient rounding, within a collaborative, physician-led environment.

nC3 is an innovative, physician-led cardiovascular practice founded by Dr. Tony Das , a nationally recognized interventional cardiologist. Our practice delivers comprehensive cardiovascular care across coronary, vascular, structural heart, electrophysiology, heart failure, and preventive cardiology.

nnDr. Imran Baig – Interventional & Structural Cardiology

nDr. Nicholas C. Macpherson – Cardiovascular Imaging & Heart Failure

nDr. Azizul Rehman – Interventional Cardiology

nDr. Ken Johnson – Interventional Cardiology

nDr. Syed Saad – Electrophysiology

nDr. Shirely Reed – Non-Invasive Cardiology

n nYou will work closely with a highly experienced clinical team, including advanced practice providers and dedicated support staff.

nPosition Overview

nnSchedule: Monday–Thursday 8:30 AM – 5:00 PM, Friday 8:30 AM – 3:00 PM

nLocations: Weekly rotation between Dallas and Heath clinics

nHospital Rounding: Light inpatient rounding across Dallas, Plano, Rockwall, and Rowlett

nCall: 1:4 weekend rotation

nEMR: eClinicalWorks (outpatient) and Epic (inpatient)

n nThis is primarily an outpatient role with a manageable inpatient component and strong clinical support.

nKey Responsibilities

nnEvaluate, diagnose, and manage cardiology patients in the outpatient setting

nPerform and document history, physical exams, and follow-up care in the EMR

nReview and communicate diagnostic testing results, including imaging

nCollaborate with physicians, APPs, and clinical staff on patient care plans

nTriage patient needs, including messaging, pre-operative clearance, and peer-to-peer reviews

nProvide oversight for in-office testing (CT, PET CT, nuclear stress testing)

n nQualifications

nnNP or PA from an accredited program

nNurse Practitioners: AGACNP required

nMinimum 5 years of cardiology experience required

nActive Texas license in good standing

nDEA and Controlled Substance Registration

nACLS and BLS certification
