nUnityPoint Clinic is a physician-led, nonprofit health care organization offering comprehensive services in primary care, walk-in care and specialty care. The clinical teams at the area’s clinics are improving outcomes, lowering the number of hospital readmissions and reducing avoidable costs associated with treating chronic conditions. nnnEstablished outpatient practice located at UPH Prairie Parkway medical Campus
n
nnJoin team of 7 Physicians and 6 Advanced Practice Providers
n
nnFull-time position, including dedicated administrative time, with flexible scheduling to ensure sufficient patient access
n
nnPrairie Parkway provides patient-centered and comprehensive care in one 90,000 square foot building.
n
nnPatients have access to therapy, lab, pharmacy, and X-ray onsite
n
nnPrimary and specialty providers at the medical campus work collaboratively and include (Rheumatology, Pediatrics, OBGYN, IM, and Diabetes and Endocrinology)
n
nnCohesive group of colleagues and supportive staff allowing for autonomy while building the practice according to your preferences
n
nnExperienced staff, both clinical and non-clinical, who know the patients and the community
n
nnFully integrated system with clinics and hospitals on EPIC EMR to improve care coordination
n
n nResponsibilities
nnnClinic hours vary from 7AM-5PM and extended hours on Monday and Wednesday 7AM- 7PM
n
nnAverage 18-22 patients per day
n
nnAbility to work as a team member to improve the health of the people and communities we serve
n
nnShared clinic call amongst the providers. Call is light, and first triaged through MyNurse line
n
nnGeneral family medicine procedures such as joint injections, skin excisions, lacerations, casting, joint injections etc.
n
nnEvaluates, interprets and coordinates information to make decisions and recommendations regarding patient care
n
nnPatient-focused, friendly and willing to collaborate in a team environment
n
nnExcellent communication (written and verbal), analytical and problem-solving skills
n
n nQualifications
nnnExperienced provider preferred
n
nnNurse Practitioner or Physician Assistant
n
nnBoard Certified
n
nnHold an unrestricted medical license in the state of Iowa
n
nnHold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice
n
