Job description
Optum Home & Community Care is hiring a HouseCalls Advanced Practice Clinician (NP or PA) to conduct annual in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with PCPs, with no prescribing required.
We offer flexible scheduling, 20–40 hours per week, 8am–8pm across a 7-day week, with no on-call, weekends, or holidays.
