n nResponsibilities
nnnProviding medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician
n
nnCompleting history and physical examinations by evaluating the patient's medical condition and health history
n
nnCompleting the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based
n
nnMaking recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
n
nnCompleting and maintaining accurate clinical notes
n
nnCommunicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers
n
nnEnsuring compliance with all state and federal legal and regulatory requirements
n
nnAdditional responsibilities as assigned
n
n nQualifications
nnnGraduate from an accredited School of Nursing and accredited Nurse Practitioner program
n
nnCurrent DEA license, if applicable to state regulations
n
nnCurrent nursing license in state of practice
n
nnCurrent CPR certification
n
nnMaster’s degree with minimum of one year NP experience
n
nnOne year hospice or palliative care experience preferred
n
nnAccreditation from an approved certifying body for advanced practice nursing as required by state of practice
n
nnCertification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics)
n
nnCommitment to clinical and documentation excellence
n
