HarmonyCares, a physician-led provider of home-based primary and palliative care, is seeking an Advanced Practice Provider to serve medically complex, home-limited patients. In this role, you will conduct in-home and telehealth visits, perform comprehensive assessments, manage chronic conditions, and support palliative care needs. You will collaborate closely with an interdisciplinary team to coordinate care plans, reduce avoidable hospitalizations, and improve outcomes.
