nnJob Description:nYour Impact:
Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home.
Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients.
Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions.
Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.
nnJob Requirements:nWhat You Bring:
Advanced Degree: Master’s Degree (NP) with a preferred focus on Adult, Acute Care, Geriatric, and/or Family.
State Required Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP).
Reliable Mobility: Valid driver’s license and vehicle for local field-based travel.

Drug Screening Requirement
As a condition of employment, WellBe Senior Medical requires all candidates to successfully complete a pre-employment drug screening.
