Pay Rate: $100/hr

Shift: Per Diem (Days)

Job Overview: As a Nurse Practitioner in the Home Health unit, you will play a vital role in providing comprehensive healthcare services to patients in their homes. This fee-for-service position involves serving as a NY Medicaid Provider and requires ANCC certification. Key Responsibilities:

Conduct patient assessments, diagnose medical conditions, and develop appropriate care plans.

Collaborate with a multidisciplinary team to ensure comprehensive and coordinated patient care.

Provide education and support to patients and their families regarding health management and preventive care.

Administer prescribed medications and treatments.

Maintain accurate and up-to-date patient records.

Qualifications:

Nurse Practitioner (NP) degree

ANCC Certification

Valid NY Medicaid Provider status

Previous experience in home health preferred

Strong communication and interpersonal skills

Working Conditions:

Per Diem position with flexible scheduling
