Job description
Matrix Medical Network in the United States is seeking a qualified NP/PA clinician to perform Comprehensive Health Assessments (CHA) in members' homes or SNFs. Each visit lasts up to an hour, reviewing medical history, medications, and social risk factors to improve care gaps.
Providers work with PCPs and care teams, travel across states, and receive a competitive base salary plus incentive compensation. Benefits include mileage reimbursement, PTO, 401(k), CE time/reimbursement, and more.
