Job description
Signify Health Medical Associates, PLLC is seeking a Part-time Clinician (NP or PA) to perform In-Home Health Evaluations, visiting members at their homes and bridging gaps in care. The role offers flexible local travel, in-home assessments, and no prescription requirements.
You’ll evaluate health history, perform exams, and use an iPad workflow to document visits. Hours are approximately 29.5 per week with competitive pay and benefits for part-time staff.
