Job description
ANA-Ohio seeks a qualified healthcare provider to deliver comprehensive primary care to veterans in a defined panel. The role leads a Patient Aligned Care Team (PACT) and collaborates with a multidisciplinary staff to ensure quality, coordinated care.
The incumbent will perform history taking, examinations, labs, and treatment planning across acute and chronic conditions, with documentation in patient records, and may support other duties as needed. Telework is eligible under VA policies.
