Nurse Practitioner
Carease Health a nationwide preventative healthcare company, is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team of dedicated healthcare professionals. As a Nurse Practitioner with Carease Health, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities.
About the Role:
As a hybrid Nurse Practitioner, you will combine on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Your success in this role will depend on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.
Responsibilities:
Provide high-quality, patient-centered care to residents in skilled nursing facilities
Conduct on-site visits and remote work from home, supported by a dedicated scribe team
Collaborate with healthcare teams to develop and implement individualized care plans
Maintain accurate and thorough documentation of patient care
Participate in quality improvement initiatives to minimize hospitalizations and improve patient outcomes
Requirements:
Active NP or PA license (in the state of practice)
1+ year of experience in Geriatric care and documentation review
Excellent communication, collaboration, and teamwork skills
