NURSE PRACTITIONER (NP) - IN-HOME HEALTH RISK ASSESSMENTS
Position: PRN / 1099 Contractor
Exclusions: PMHNP, Women's Health, Pediatric NP's and PA's.
Schedule: Flexible (50+ hours/month)
Compensation: $105 - $115 per visit
$100 - 50-60 hours per month
$110 - 80+ hours per month
$115 - 90+ hours per month
This is a pure assessment role: No prescribing, no on-call, and zero follow-up care.
THE PAY BREAKDOWN
$115 - 50-60 hours per month
$120 - 70-80 hours per month
$125 - 80+ hours per month
Additional Pay: $250 flat-rate paid training
Additional Pay: $25 full HRA no-shows
Additional Pay: $65 diagnostic-only visits
Additional Pay: $10 diagnostic no-shows
Fast-Track Bonus: Extra incentive bonus if you complete onboarding paperwork within 24 hours
WHAT YOU'LL DO
Conduct in-home HRAs (medical history, lifestyle, basic physical/mental evaluations).
Provide preventive care education.
Document visits using a company-provided iPad.
COUNTY COVERAGE AREAS
(Must be willing to cover both counties, but not all in the same day.)
Cleveland | Lorain | Medina | Mentor | Akron | Sandusky
