Title: Primary Care Nurse Practitioner
nProgram: Healthcare Department, Mobile Clinics
nWork Location: Varies; sites are located in the Bronx, Manhattan, and Brooklyn
nStatus: Part Time
nSchedule: Monday, Thursday, and Friday 9-5
nReports to: Medical Director, Primary Care
nSalary: $80,000 to $87,000 depending on experience
nPosition Overview:
nThe Primary Care Nurse Practitioner working in Mobile Health is responsible for the examination, diagnosis and the treatment of patients within the scope of the Health Center. It is preferred the candidate has experience working with patients experiencing homelessness, as well as those with substance use disorders and serious mental illness. Experience providing care in a mobile setting is a plus. This posting is for a part time Family Nurse Practitioner, working three days a week.
nEssential Duties & Responsibilities:
nThe essential duties of the Primary Care Nurse Practitioner include but are not limited to the following activities:
nnnProvides medical diagnoses, treatment and advice to patients in accordance with New York State regulations and the scope of training of the clinician.
n
nnResponsible for consulting with the appropriate medical specialists, when deemed necessary according to the Health Center protocol.
n
nnPrescribes and/or dispenses required medication to patients
n
nnProvides medical supervision to the assigned clinical team leaders
n
nnDirects, organizes and participates in preventive health programs
n
nnParticipates in the development and implementation of quality assurance management and utilization review
n
nnParticipates in interdisciplinary meetings with shelter and housing staff for the assigned site to discuss high risk patients
n
nnParticipates in quality improvement initiatives
n
nnParticipates in community outreach programs as needed
n
nnContributes to the overall functioning of the department
n
nnMaintains patient records in accordance with the policies of the department
n
nnMaintains all New York State and federal licensing requirements as current according with the department’s credentialing policy
n
nnMaintains current certification in Basic Life Support
n
nnCompletes 30 annual Continuing Medical Education credits.
n
nnParticipates in Departmental meetings and conferences.
n
nnParticipates in on call coverage on a rotating basis, taking a total of 4 weeks of telephone call coverage per year.
n
n nQualifications:
nnnNew York State NP License and Registration
n
nnFamily Nurse Practitioner Board Certification
n
nnExperience in providing services to homeless and substance use population preferred; mobile health experience is a plus
n
nnDemonstrated knowledge of health related concerns and experience in providing medical care in urban communities
n
nnCommitment to community-based health care services
n
nnBilingual (Spanish/English) preferred
n
