Nurse Practitioner (NP/APRN) – Mobile Primary Care
Full-Time | Monday-Friday
$100,000 - $110,000 Annual Salary (with the average NP making $154K/year)

Mobile Medical is seeking a compassionate and independent Nurse Practitioner (NP/APRN) to provide in-home primary care throughout West Duval County, FL.

Position Overview
As a Mobile Medical Nurse Practitioner, you will provide primary care services to patients in their homes throughoutWest Duval County, FL.
This role is ideal for experienced Nurse Practitioners who enjoy working independently, managing chronic conditions, and developing long-term relationships with patients in a community-based setting.
Primary Responsibilities
Perform comprehensive in-home health assessments
Diagnose and treat acute and chronic medical conditions
Develop individualized treatment and care plans
Order, review, and interpret laboratory and diagnostic testing
Prescribe medications and monitor treatment effectiveness
Educate patients and caregivers regarding disease management and preventive care
Coordinate care with physicians, specialists, home health agencies, and interdisciplinary teams
Maintain accurate and timely electronic medical records documentation
Support preventive care and population health initiatives
Qualifications
Required
Master's Degree from an accredited Nurse Practitioner program
National Board Certification as: Family Nurse Practitioner (FNP)
Adult-Gerontology Nurse Practitioner (AGNP)
Adult Nurse Practitioner (ANP)
Current DEA Registration (or ability to obtain)
Current Professional Liability Insurance
Valid Florida Driver's License and reliable transportation
Preferred
Previous experience as a Nurse Practitioner in: Primary Care
Internal Medicine
Family Practice
Home Health
Mobile Medical
Geriatrics

Post-Acute Care
Strong clinical assessment and diagnostic skills
Experience managing chronic disease populations
Ability to work independently and efficiently in the field
