Key Responsibilities

Delivering thorough patient evaluations, requesting diagnostic tests, and crafting personalized care management plans

Diagnosing and managing acute, chronic, or episodic health conditions either in collaboration with physicians or independently (depending on credentialing)

Providing education to patients and families on health maintenance, prevention, and chronic disease management

Prescribing medications and treatments in alignment with your scope of practice

Engaging in ongoing patient follow-up and chronic care initiatives

Working cohesively within interdisciplinary teams to guarantee comprehensive, seamless patient care

Maintaining accurate and thorough records within electronic medical record systems (EMR)

Educational Qualifications

Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)

Graduation from a recognized Nurse Practitioner program

Licensure and Certifications Required

Valid Nurse Practitioner license in the state of Illinois

National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or similar credentials

Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or ability to secure them within 90 days)

Essential Skills
