Your Role in the Community

Carrying out thorough patient evaluations, ordering necessary tests, and crafting personalized care routes

Identifying and managing acute and chronic conditions, either independently or in collaboration with physicians

Providing education to patients and their families on wellness, prevention, and disease management strategies

Prescribing medications and therapies aligned with your scope of practice

Engaging in patient follow-ups and chronic care management

Collaborating with multidisciplinary teams ensuring comprehensive care

Accurately documenting all patient information within electronic medical records (EMR)

Qualifications You Need

Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) credentials

Completion of a recognized Nurse Practitioner program

Essential Certifications

Current Nurse Practitioner license in the state of Colorado

National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or equivalent specialty

Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or willingness to obtain within 90 days)
