Maintain accurate and timely clinical documentation.
Support quality measures, risk adjustment, and appropriate HCC documentation.
Participate in interdisciplinary care meetings and clinical improvement initiatives.
Provide occasional telehealth or other visit modalities based on patient needs.
Participate in a shared after-hours coverage rotation.
Help support and mentor new clinicians or NP students when appropriate.
