Nurse Practitioner (NP) - Home Health Care (Northern New Jersey)
Salary: $130,000 - $140,000 per year

We are seeking a reliable, compassionate, and selfsufficient Nurse Practitioner to support homebound patients throughout Northern New Jersey. This role is handson, relationshipfocused, and allows you to provide consistent, highquality care directly where patients need it most.
What You'll Be Doing
You'll be the primary provider for 5-6 homebound patients per day, delivering highquality care in the comfort of their homes:
Perform assessments, vitals, and physical exams
Diagnose health conditions and illnesses
Prescribe and manage medications
Maintain accurate clinical notes and visit schedules
Build trust and provide guidance to patients and families
Educate on conditions, prevention, and overall health habits
Promote patient-centered, compassionate care
What You Need Active New Jersey NP license
DEA license
Malpractice coverage
Strong communication skills and professionalism
Ability to work independently
Proven NP experience in primary care, ER, clinic, or nursing home settings
Salary $130,000 - $140,000 per year
