NURSE PRACTITIONER NP - HOSPICE & PALLIATIVE CARE SERVICES

nnnProactive, collaborative team member in a hospice care environment

nnConducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care

nnCommitment to caring for patients and partners

nnComplete all documentation and records in a timely and accurate manner
n
nnFollow established policies and procedures as related to the scope of practice
n
nnMaintain continuity of patient care in a hospice setting

nLICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS
nnnCurrent, active, and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in state of practice

nnBoard-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care
n
