Key Responsibilities
Performing thorough patient evaluations, ordering necessary diagnostic procedures, and crafting personalized treatment strategies
Identifying and managing acute, chronic, or episodic health issues in collaboration with physicians or autonomously (subject to credentialing)
Providing education to patients and their families regarding health promotion, prevention strategies, and chronic disease management
Prescribing medications and therapies as per the scope of practice
Engaging in patient follow-ups and chronic care coordination
Working cohesively with interdisciplinary teams to deliver comprehensive and integrated care
Maintaining thorough and accurate documentation within electronic medical records (EMR)
Qualifications
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
Completion of an accredited Nurse Practitioner educational program
Certifications Required
Active Nurse Practitioner licensure in New Jersey
National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or a recognized specialty equivalent
Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or the capacity to obtain within 90 days)
Essential Skills
Strong clinical judgment complemented by a compassionate approach to patient care
Excellent interpersonal skills with patients, families, and healthcare teams
Flexibility in dynamic, patient-focused environments
Proficiency with EMR systems and familiarity with telehealth solutions
