Job Title: Nurse Practitioner (NP) / Physician Assistant (PA)

We are seeking a highly skilled and dedicated Nurse Practitioner (NP) / Physician Assistant (PA) to join our team. As a provider with our organization, you will have the opportunity to make a meaningful impact on the lives of our residents while working in a fast-paced and dynamic environment.
Key Responsibilities:
Provide high-quality medical care to residents in a SNF setting, focusing on reducing unnecessary hospitalizations and promoting effective treatment in place
Conduct thorough assessments, develop individualized care plans, and order necessary treatments and medications
Collaborate with facility staff, including physicians, nurses, and other healthcare professionals to ensure seamless continuity of care
Communicate effectively with families, residents, and healthcare providers to ensure open lines of communication
Stay up-to-date with industry best practices, regulations, and survey processes to ensure compliance and continuous improvement
Requirements:
Salary: $105,000 - $152,000 ANNUALLY
