Optimal Wound Care is looking for an experienced Nurse Practitioner with a passion for wound care to join our growing mobile wound-care team.
We provide specialized wound care directly to patients in their homes, assisted living communities, rehabilitation centers, and post-acute care settings . Our goal is simple: provide high-quality, specialized wound management where patients need it most.
This is a great opportunity for an NP who enjoys autonomous clinical practice, building relationships with patients and care teams, and treating complex wounds in a mobile setting.
What You'll Do
Perform comprehensive wound assessments and develop individualized treatment plans
Evaluate and treat chronic, acute, and non-healing wounds
Perform wound debridement and other advanced wound-care procedures
Manage diabetic foot ulcers, pressure injuries, venous and arterial ulcers, surgical wounds, and sacral wounds
Utilize advanced wound-care therapies and grafts when clinically appropriate
Monitor wound progression and adjust treatment plans based on patient response
Collaborate with physicians, home health nurses, case managers, assisted living teams, and other healthcare professionals
Provide education to patients, families, caregivers, and clinical teams
Document assessments, treatment plans, procedures, and follow-up care accurately
Help identify complications early and support efforts to reduce avoidable hospitalizations
What We're Looking For
Current Nurse Practitioner (NP/CRNP) license in applicable states
Board certification as a Nurse Practitioner
Wound-care experience strongly preferred
Experience with chronic, complex, diabetic, pressure, venous, arterial, or post-surgical wounds
Experience performing wound debridement preferred
Strong clinical assessment and decision-making skills
Comfortable working independently in a mobile/community-based environment
Excellent communication and documentation skills
Ability to collaborate effectively with multidisciplinary care teams
Valid driver's license and reliable transportation
