Responsibilities You Will Embrace
Performing thorough patient evaluations, recommending diagnostic tests, and developing personalized care strategies
Identifying and managing acute, episodic, or chronic conditions in partnership with physicians or independently (as per credentialing)
Instructing patients and families on preventive care, wellness strategies, and chronic disease management
Prescribing medications and therapies aligned with your professional scope
Engaging in patient follow-up and holistic chronic care management
Partnering with interdisciplinary teams to provide seamless, comprehensive care
Meticulously documenting patient interactions within electronic medical records (EMR)
Education Requirements
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
Completion of an accredited Nurse Practitioner program
Required Certifications
Active Nurse Practitioner license in the state of New York
National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or specialty equivalent
Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or ability to obtain within 90 days)
