Job Title: Advanced Practice Wound Care Nurse Practitioner (AGNP/CNP) | Geriatric Rounding

Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

Benefits: Full Benefits Package, PTO, Paid Holidays, CME Allowance, Malpractice Coverage, DEA Reimbursement, and More

Schedule: Full-Time | Monday through Friday

Most healthcare providers rarely get to see the full outcome of their work.

A referral is made.

A treatment plan is started.

Then the patient disappears into the healthcare system.

Wound care is different.

You see the progress.

You watch healing occur.

You adjust the plan.

You witness the results of your clinical decisions unfold over time.

At Twin Cities Physicians, we're seeking a Nurse Practitioner who enjoys seeing measurable outcomes and wants to play a direct role in helping patients heal.

Our providers deliver care directly inside assisted living communities, memory care settings, skilled nursing facilities, and long-term care communities throughout the Twin Cities Metro. This role combines advanced wound care, geriatric medicine, chronic disease management, and community-based rounding into one highly rewarding clinical opportunity.

Organizations such as Bluestone Physician Services, Genevive, and Lifespark have helped shape this evolving model of care. Twin Cities Physicians continues to expand throughout Minnesota by bringing specialized healthcare directly to senior communities.

Why Providers Find This Work Rewarding

$120,000 to $160,000+ total compensation opportunity through a combination of salary and productivity incentives.

Follow patients over time and see the tangible results of your treatment plans.

Develop expertise managing pressure injuries, diabetic wounds, vascular wounds, post-surgical wounds, and other complex conditions.

Join a growing provider organization serving senior communities throughout Minnesota.

Receive mileage reimbursement, CME support, malpractice coverage, DEA reimbursement, PTO, and comprehensive benefits.

Work alongside experienced physicians, nurse practitioners, and interdisciplinary care teams focused on improving outcomes.

What You'll Be Doing

Provide advanced wound care assessments, treatment planning, and ongoing wound management.

Conduct routine rounding visits within assisted living, memory care, skilled nursing, and long-term care communities.

Manage medications, diagnostics, treatment plans, and follow-up care.

Collaborate with physicians, nurses, therapists, caregivers, and community staff.

Support chronic disease management and continuity of care initiatives.

Help improve healing outcomes while reducing avoidable complications and hospitalizations.

Qualifications

Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).

Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred.

Strong clinical judgment and confidence managing medically complex patients.

Valid driver's license and ability to travel throughout the Twin Cities Metro Area.

Some providers spend their careers hoping their interventions worked.

In this role, you'll often get to see the results for yourself.

If you're looking for a full-time opportunity that combines advanced clinical care, visible patient progress, and the chance to make a lasting difference in senior communities, we'd love to connect.
