Job Duties
nUniversity of Minnesota Physicians is seeking a Certified Nurse Practitioner or Certified Physician Assistant to join our Cardiovascular Genetics practice at the M Health Fairview Clinics and Surgery Center in Minneapolis.
nIn This Role, You Will:
nWork in the ambulatory space to provide comprehensive, longitudinal ambulatory care for adults with inherited or suspected genetic cardiovascular conditions, including cardiomyopathies, inherited arrhythmia syndromes, aortopathies, and other heritable cardiovascular diseases.
nPerform new and follow-up patient evaluations, including comprehensive histories, physical assessments, review of diagnostic testing, and development of individualized plans of care in collaboration with the Cardiovascular Genetics team.
nProvide ongoing management and surveillance for patients with inherited cardiovascular disease, including coordination of appropriate cardiac imaging, rhythm monitoring, laboratory testing, and other diagnostic evaluations.
nWork closely with cardiovascular genetic counselors and physician specialists in the evaluation of patients and families with known or suspected inherited cardiovascular disease.
nParticipate in the review and interpretation of genetic testing results within the APP's scope of practice and in collaboration with genetic counselors and physicians, incorporating results into individualized cardiovascular management, surveillance plans, and the importance of appropriate screening for at-risk family members.
nReview and incorporate multigenerational family histories and pedigrees into cardiovascular risk assessment and clinical decision-making.
nHelp identify patients who may benefit from genetic evaluation and facilitate appropriate referrals for genetic counseling and testing.
nCollaborate closely with cardiovascular genetics, genetic counseling, electrophysiology, heart failure, cardiovascular imaging, vascular/aortic specialists, and other multidisciplinary partners to provide coordinated, patient-centered care.
nParticipate and help lead monthly Cardiovascular Genetics Multidisciplinary team meetings.
nCommunicate recommendations and plans of care with referring physicians, primary care providers, and outside institutions to promote continuity of care.
nProvide General Cardiology care, including evaluation and longitudinal management of cardiovascular conditions with an emphasis on cardiac sarcoidosis.
nSupport timely patient access through independent APP clinics and collaborative physician/APP practice.
nParticipate in the continued development and growth of the Cardiovascular Genetics program, including opportunities to help develop clinical workflows, care pathways, patient education, and best practices for inherited cardiovascular disease.
nDemonstrate a commitment to exemplary patient care, evidence-based practice, and appropriate utilization of healthcare resources.
nDeliver care individualized to the developmental, cultural, and clinical needs of the population served.
nProvide occasional cross-coverage for inpatient General Cardiology based on clinical and operational needs.
nUtilize a fully integrated Electronic Medical Record (Epic) for documentation, communication, and coordination of care.
nPossible outreach sites of Maple Grove or Fridley.
nYour Schedule:
n1.0 FTE
n10-hour workdays Monday-Friday
nNo nights or weekends
nWe're Looking For a PA or NP Who Is:
nA graduate of an accredited Physician Assistant or formal Adult Nurse Practitioner training program.
nEligible or is certified through NCCPA, ANCC or AANP.
nLicensed or eligible to acquire a license and prescriptive authority through the MN Board of Medical Practice (PAs) or MN APRN license or eligible to acquire a license and prescriptive authority through MN Board of Nursing (NP's).
nAble to meet threshold criteria/qualifications for credentialing and privileges.

nThe full-time salary range is $130,866-$161,954 commensurate with practice experience and may also include incentive compensation or additional specialty-specific compensation components. Salary prorated based on FTE.
