Job Summary
As an Advanced Practice Clinician, you will deliver and coordinate primary care to SCAN members, focusing on independent, home‑based care to keep seniors healthy and independent.
Key Responsibilities
Provide care to an assigned panel of members (patients) in senior living communities or skilled nursing facilities, including physical exams, wellness checks, urgent visits, telephonic triage, and vaccine administration.
Order and interpret diagnostic tests, initiate goals‑of‑care conversations, and manage chronic disease.
Develop comprehensive care plans for patients, families, and community staff and coordinate with case management for necessary services.
Collaborate with primary care physicians, specialists, hospitals, and community providers to ensure continuity and shared decision‑making.
Document all visits and interactions promptly following practice protocols.
Participate in on‑call coverage after hours and weekends; engage in virtual visits to triage and reduce drive time.
Support integrated care sales events to build member awareness and educate communities.
Contribute to team efforts to close care gaps, attend risk‑group discussions, and optimize clinical workflows.
Qualifications
Master’s degree in Nursing or an accredited PA program; adult/geriatric focus preferred.
Current California Nurse Practitioner license (NP) or Physician Assistant certification (PA‑C) in good standing.
Wound Care Certificate and Palliative Care Certificate are a plus.
Knowledge of complex chronic diseases (e.g., dementia, diabetes, heart failure, COPD) and HCC/ICD‑10 coding.
Understanding of CMS guidelines, Medicare Advantage Managed Care, and HIPAA requirements.
Strong interpersonal skills with a change‑agent mindset; ability to build relationships with teams and patients.
Basic proficiency with Word, Excel, PowerPoint, Outlook, and EMR; openness to new technologies.
Physical ability: stoop, kneel, crouch, bend, reach, lift, pull, push, and carry up to 10lb regularly and up to 50lb occasionally.
Multilingual or multicultural experience is a plus.
Valid driver’s license, automobile insurance, and reliable transportation for travel of 75% of the time within the assigned area.
Travel and Physical Requirements
Travel 75% of the time within the assigned region.
Physical demands include stooping, kneeling, crouching, bending, reaching, lifting (10–50lb), and moving between patient homes.
